Played my friends SM last night. He used it twice and it scattered both timed before he landed it the 3rd time. Is this a correct use of it? 1 direct hit per game or 1 attempt per game?

No, he is only allowed to use it once per game, regardless of where it lands. Look at the chapter master entry on p52 of his SM codex and you'll see it clearly stated.

And the Chapter Master cannot move in the Movement phase before he is going to use it.
