The Red Thirst is when the BA get an overwhelming bloodlust and charge into CC and the Black Rage is when they become DC and are beyond any form of control.

The Red Thirst continually plagues Blood Angels, but they can recover some measure of self control. The Black Rage is a psychotic/psychic episode during which they believe themselves to be Sanguinius in his last moments (up to and including the final battle with Horus).

Except, who do they believe to be Horus, a great enemy leader or just the nearest enemy they see?

The enemy leader/the one who ends their life I believe, they see the enemy forces as the 'minions of Horus'.
They also see their battle-brothers as the other loyal primarchs.
