It's not an actual model, but that would be an Imperial Guardsman. He's from the Catachan regiment but they don't come with that hat

it's from the new catachan command squad set.

The kneeling legs and open hand arm are from the Catachan gun teams set, the head, body, bolter arm are from the Catachan command set. My Catachan colonel uses that absolutely awesome beret head:
