I notice some mistakes and have some question of my own:
1. If you take notice at a small planet located in Segmentum Ultima, on the left side of Gates of Varl. You will see imperial world Dolumar, if I am not mistaken this is where plot from "Fire Warrior" is happening. How the hell Tau get so far away from home? ( Tau Empire is located on the far right of the map ).
2. What is XCV-104? Necron Tomp World?
3. Why is majority of Segmentum Ultima unsettled?
4. Why are 2 Ork planets in Segmentum Solar? I thought that Imperium has full control over it ( being that Sol system is in it )?
5. Are Q'orl extinct? ( Lexicanum said that they "where" a threat to the Imperium ).
6. What is Dyson sphere? ( above the Perseus Arm, lover right of the map ).
7. Above the Hive Fleet Leviathan name you can see Orion Arm. When you point your mouse on it it said that the Earth's solar system is close to the inner rim. I thought that Earth is Holly Terra in 40k?
8. If Genestealers are from Tyranids Hive fleets how can they have home-world in a Milky Way? ( the world Ymgarl, left from the Ultramar, right side of the map )

2. Some worlds found don't get a name, merely a designation. It could also be a research station or a mining works on an asteroid. 4.The Orks are widespread and hard to eradicate, Armageddon is within segmentum solar and that is under attack all the time. 5. It may be that they are trying to actively avoid the Imperium or have merely ceased to be a large military threat. 6.The Dyson Sphere is rumoured to contain the Outsider one of the few c'tan left. 7.It is afaik 8.Genestealers were around long before the Tyranids were, there are many species attributed to being vanguard organisms from the Tyranids genus including the Fenrisian Kraken and the Catachan devil. I cannot help with the others... Number 1 is likely to be a mistake. Number 3 i have no idea about.

Generally this Map is nice and shows some locations from background storys. But it isn't perfect, isn't complete.

6- A Dyson sphere is actually an artificial construct surrounding a star to tap it's energy. Due to the nature of the necron, it sounds like using it as a cage for a necron god is very likely.
3- I've always thought of Segmentum Ultima like the Wild West of American History. It's the 'wild frontier' of space. There are settlements but they are relatively few and far between.
8- Ymgarl was thought to be the home of the creature known as the genestealers. It was later discovered that genestealers were a scouting organism of the vast hive fleets. Perhaps they were launched
into the Milky Way to act as an advanced scouting force to lure the Hive Fleets to an all you can eat buffet of biomass.

To totally remove them yeah probably exterminatus or something of that magnitude.
You can kill them all and conduct regular firesweeps to keep their population down.

I thought I'd just throw this in. This is NASA's most recent estimation of the galaxy from the top down. Be nice if someone made a swell, if not 'accurate' map using this.

Don't forget, Metalica is a forge world, home to the Legio Metalica Titan Legion. I think the win factor just increased here....
As for the initial questions....
1. Somebody dun goofed when they made the map?
2. It's probably an Ork world, given its proximity to Gorkamorka and several other Ork worlds.
3. Cause it's the furthest away from Earth, most likely. Humans settled the Segmentums Tempestus, Pacificus and Solar the most heavily. Not to metnion, this map is FAR from complete. The Imperium has a millioin worlds alone, remember?
4. They don't have full 100% control of it. Remember, the Imperium doesn't care about territory, it cares about worlds. Those two Ork worlds are likely well enoguh contained that the High Lords dont' think they're a threat.
5. Not sure...haven't read alot about them.
6. A Dyson sphere is a hypothetical construct that absorbs 100% of the energy released by a star. Dyson Spheres are considered something to look for when searching for high advanced alien life, as they are ways a species can tap the huge amount of energy that a star provides.
7. It is. However, Earth is in the Orion Arm. Go to the center of the Milky Way (around where the Gates of Varl are) and trace that think arm of stars that goes through the segmentum solar. THAT is the Orion Arm.
8. Imperial Biologists used to believe the Genestealers were just another weird Xenos, this time from Ymgarl....when it was revealed that Genestealers were, in fact, Tyranid shock troops, it was quite a shock.

Afaik The Deciever tricked him into doing it when the Nightbringer was turning friendly fire into a mix between a sport and a buffet.

Unkonw, we're never given any information about the locatoin of Kaurava or Kronus, other than that they're close enough to the Tau Empire for the Tau to BE there, so they're definitely off in the Ultima Segmentum somewhere.
Subsector Aurelia and Lorn V are both close enough that Cadian regiments were sent there, so I'd put them somewhere in the Segmentum Obscurus.

Ok, to put everything in perspective, that map isn't showing the Milky Way in the traditional orientation(Galactic North being up)
in this case, the map has the Galactic West(the side Terra is located on) at the bottom side.
1) likely a mistake, or possably a world with the same or a similer name.
2) as said before, planets are given a identification number. Planets are only identified by their names if they actually have been given one by their inhabitants, but for Imperial Tithe collection they have a number. sort of like a Social Security Number for Planets.
3) the Great Crusade had only gotten part of the way into Ultima Segmentum before the Heresy happened. as a result, the vast majority of the Segmentum is unmapped. possably containing whole Human and Xeno Empires as yet undiscovered.
4) Warp Travel isn't linear in it's distance of travel. it is possable, due to Warp Turbulance, that a world 50 Light Years away could take many years to reach while a world accross the Galaxy could take only a few weeks.
6) the Dyson Sphere was belived to have been built by the Outsider under the influence of the Deciever. the Outsider was then trapped in the prison of his own construction. he has since gone quite mad trying to escape.
7) this is because the Orion Arm of the Milky Way has the Sol(Earth) system in it. the Arm streaches accross several segmentums, but they placed the name of the arm in a place where there wasn't any planets in the way.
8) Ymgarl was the first place of contact with Genestealers by the IoM. this was prior to the Tyranid invasion and as such weren't identified as being the same thing. consequently, the Varity of Genestealers with the ability to shape shift their bodies were called Ymgarl Genestealers.
