Do the plague marine champions have special names/ title? are they divided into 'chapters' so to speak? if so, do these 'chapters' have names? Do their tanks have cool names?

None of the Legions divided into official chapters afaik though chapter was a term that refered to 1000 marines during the time of the legions. It is possible that 1000 Deathguard have formed a warband which could be called a chapter. Their tanks can have cool names if you give them cool names... The chamions can have cool names/titles if you give them cool names/titles... A warband would probably have a name... Most Deathguard ones are related to disease, plague... you know Nurgle stuff.

Some of the legions, such as the Word Bearers and Ultramarines, had formations called Chapters. They had companies within those chapters.
However, the Death Guard didn't go that detailed into their Legion makeup. They had simply 7 Great Companies. They also had 3 titles of honor. First was the First Captain, the Second Company captain was known as Commander, and the 7th Company captain was known as the Battle-Captain.

While the DeathGuard has no official chapters there exist The Purge, Lords of Decay, and the warband of Necrosius, all breakaway factions known to ally when circumstances call for it. The Cleaved may also be a Deathuard warband though I am unsure.
Update: the band lead by Necrosius is called the Apostles of Contagion and I've found the name of another band called The Tainted, who seem to be followers of the Daemon Prince Mamon.
