Having read Nightbringer of the ultrmarines omni, I'd go for the Haemonculus. They may be the most evil sadists imaginable, but the Dark Eldar medical science seems pretty immense.

They strip out one of the characters nervous system and replace it with a new one...

Haemonculus. Ork surgery only works due to the hardiness of the ork physiology, so choosing a painboy is like choosing to be butchered and killed.

With a Haemonculus at least you have a chance that he decides to give you super powers.

I think I'd trust the Haemy on the hope that he'd actually spare me the pain... as if. The Painboy would chop off the wrong limb and give me a shooty one instead That and, no anesthetic :S

Haemonculus fo sho. They actually have great medical knowledge...they just use it to inflict as much pain as possible.

My big worry about the Haemonculus route is the following situation:

I'm just drugged enough not to risk death by pain, but otherwise conscious to a room of weirdos masterbating over me as they figure out new wackiness to inflict.
