Actually....the Monolith rule says that when teleporting through a Monolith, you treat it as having disembarked from a stationary vehicle.
So regardless of what the monolith has done, the teleportation is treated as having disembarked from a stationary vehicle - so models having teleported can still assault.
However, the
FAQ made a clarification noting that a teleporting unit cannot move after teleporting through a monolith that has already moved. IE, you cannot move the monolith, teleport a unit, then move again with the unit.
You can however move a monolith, move a unit, then teleport - after which you are free to assault, just like having disembarked from a stationary vehicle.
You can also teleport a unit, move them, and then move the monolith.

And no - they are considered disembarking from a stationary vehicle, with the caveat that a teleporting unit may not move again after teleporting through a monolith that has already moved.
The following are allowed:
Move, teleport, shoot, assault.
Teleport, move, shoot, assault.
The caveat that you can't move if you have already moved or if the monolith has moved is to prevent abuse. Imagine this: Move, teleport (stationary vehicle), move again, shoot, assault. Or to gain extra inches the same way: Move monolith forward, teleport unit, move unit forward additional 6", shoot, assault.
The rules are in place to only let you move, shoot and assault the normal distance you would be allowed to do so, albeit from a different starting point (via teleportation).
