I think the combi melta model only comes in the sternguard and captain kits. The best way to do it is just make your own. You would have a hard time finding them in any quantity online because they are quite sort after.

The best thing you can do is convert them out of regular bolter and meltas. They do not make combi-meltas. The only model thats I know that is made with a combi-melta is the oop christmas captain.

Combi meltas, ouch. The best thing to do is make your own!
Buying them on ebay or such will be hard because they are so rare.

you can get it in plastic just........ you need to justify it:

The SM Captain has ONE but there are plenty of other use bits inside. I buy them frequently.
Before they went to failcast you could buy the combimelta Sternie in a blister.
Probably the best way is to use Infernus Pistols from the Death Company Sprue and affix them to bolters. You get two infernus pistols per DC box if you know anyone who plays BA I doubt they take many, try to get the bits. Automatically Appended Next Post: cheapest way is probably the 8 USD set of 5 meltas cut down and put on a bolter.

or vice versa (a bit easier) mount a bolter on top with side-mount magazine... might look too orky though.

Take 2 bolters. Flatten the top of one and cut of the bottome parts and the barrel of another. Glue together. Cut off the barrel bits from a Multi-Melta and cut those into 2 seperate barrels. Glue to the upper bolter section where you cut of the bolter barrels.
Its not the prettiest conversion, but it passes WYSIWYG muster.
You can also make combi-flamers the same way.
