Been done before, but a few new games out there that may have shown us something.
Fallout NV - Played it for a while and used ED-D and Cass as my main compainions. Downloaded Dead Money and Honest Hearts and started DM. After finishing it, I went back to Primm to get ED. Walked into the shop and he does his attack music and attacks me. I leave the shop and he follows me out and starts attacking everyone in Primm. They kill him off and I restart from the previous save and he still attacks everyone.
After a restart, I go after Cass and she starts attacking me and the same as before. I did not have Rex, so got him and fixed him and as soon as the King "gave" him to me, he is attacking everyone. This went on for a week, after Rex, I was just having fun.
Then while I was logged into Live, a patch was downloaded and the Companion Rage stopped. It was sad.
Any good ones? From any game?

I still like the one from Final Fantasy VI. You used Relm to paint Gau and the game glitched bad. After the fight you had like 99 atma weapons in your inventory plus tons of other stuff.

I have fond memories of playing Pokemon Blue and fighting Missing No. to get 99 Master Balls...
I think my favorite is the Fallout 3 glitch, where one of your companions (forgot the name) has a glitched health system where he gains health exponentially, so he basically can never die.

Is it Fawkes? I think it must be. I've never had him die in any of my games. He can get hit by rockets, mobbed by deathclaws, set off bottlecap mines and he takes barely any damage.
I can't remember any funny glitches in any games I've played for a while. I played fallout 3 again recently, funnily enough (No surprise there are bugs and glitches since it's a bethesda game) and a slightly perplexing thing happened after I passed round the junkyard where dogmeat is -- I didn't pick him up yet as I couldn't be bothered to keep him alive--, I headed northeast and came across the Roach King. I killed him and his roaches I was attacked by dogmeat and he had ridiculously, ludicrously high health. I seem to remember I had a combat shotgun, plasma rifle, SMG... I tried all of them on him, but they did barely any damage at all - like two "bars" of health. It wasn't as if I was particularly low level, either around level 10 or 11.
Maybe he was angry at me for leaving him with those raiders?

I suppose that bug must have been "introduced" with the DLC and subsequent patch(es) then because I played vanilla on my xbox and he got killed pretty easily. I was playing through the GOTY on ps3 recently, and I left him alone to start with.

There's a bug on Monster Hunter Tri, on the Desert Island, you can go "underground", and its completely messed up, like not designed messed up. You walk around and pop up in random places.

Playing Deus ex ... Not so much a glitch as bad design and the same problem i found in half life 2 (but fixed in HL2)... basically if they can't see you they won't shoot. So I upgrade my character so he can lift heavy objects and i push them in front of me ... only time i stop is to jump out from behind to knock some one out. Even more fun if you block the path the enemy doesn't even try to go down that root. I've try this with cardboard boxes and they still stop.
