Mmmm not so sure about that. Assault Ramp means you can Assault after moving. That's assuming you've got out of the vehicle, and you can't disembark after going Flat Out; and therefore can't Assault. Might be wrong (someone please verify!) but these are my thoughts on the matter.

You can assault out of the land raider after it moves due to the assault vehicle rule. If you are stationary you can disembark move up to 6" and assault as normal. If you move combat speed (0.01" to 6") you can disembark and assault as normal. If you move cruising speed (6.01" to 12") you can disembark and assault as normal. a Land raider can not move flat out as it is not a fast vehicle.

Yep. Flat out does not mean 'go at the fastest speed possible.' Many people seem to think this, but it just isn't true. Flat out moves can only be made by fast vehicles, when they move over 12". As above, LR can't flat out, so yes it can move 12" and have the contents disembark and assault.

Yes, you can.
As noted, only FAST vehicles can go "flat out" and the LR is not a fast vehicle.
