1# Death company Calistarius.
2# Cast down by Trygon against Tyranids (in codex)
3# Blood Angels omnibus when he almost fell to the BR again.

It's really not that big of a deal actually, I would assume a lot of marines will be seriously injured once or twice during their service.

I would only say 2 anyways for Mephiston (that we know of), falling to the Red Thirst (not Black Rage) isn't something I would necessarily consider being close to death.
