Before he got picked up by his pops, he was part of a gladiator ring, and I always assumed they had attached some kind of combat drug system to him that his old man was loathe to remove, for whatever reason.

The Emperor wanted to remove it. Angron, however, didn't. It only got worse when E decided to simply snatch Angron away from a fight while letting Angron's friends die.

My assumption with the face issue was a matter of artistic interpretation. It just adds to the "surgically enhanced for combat rather than appearance" angle.
