I've read that Malcador the Sigilite, being the Regent of Terra and First Lord of the Imperial Administration, was among the few people entitled to Adeptus Custodes protection apart from the Emperor Himself. But who are the others? The Council of Terra? But that doesn't make sense since the High Lords of Terra who succeeded them aren't entitled to Custode protection. The Primarchs? They used Terminators as bodyguards, and after becoming traitors, possessed. So, who are the ones entitled to Custodian bodyguards, then and now?

I should have left him there. He had served his purpose. He owed me nothing - yet he gave himself to me willingly. Why? I know not. He is nothing more than a pathetic human. An inferior race. A mon-keigh. But still I broke off my wings so that I might carry him easier. I took him from that place, into the snowstorm where our tracks will not be found. He is heavy. And he is dying. And he is slowing me down. But I will save him. Why? I know not. He is still warm. I can feel his blood ebbing across me. For every beat of his heart, another, slight spill of heat. The heat blows away on the winter wind. His blood is still warm. But fading. And I have spilled scarlet myself. The snow laps greedily at our footsteps and our lifeblood, covering them without a trace as we fade away.

'She sat on the corner, gulping the soup down, uncaring of the heat of it. They had grown more watery as of late she noted, but she wasn't about to beggar food from the Imperials or the "Bearers of the Word." Tau, despite their faults at least didn't have a kill policy for her race.'

It's from Flight of the Eisenstein, when Garro meets Malcador who tells him that the Emperor has overruled the Council of Terra's decision to treat the loyalists of the Traitor Legions as traitors as well, but instead to have Malcador find a place for them. Malcador is accompanied by a Custodian; perhaps the Council was under Custodian protection at the time - with everything that's going on - but the High Lords, I'm not so sure.

And yes, if the Emperor commands (telepathically), the Custodes will go forth. I'm sure that even the High Lords or an Inquisitor will back down when a Custodian arrives and delivers a message from the Emperor - Custodians being the only Imperials not under the authority of either the High Lords of Terra/Council of Terra and the Imperial Inquisition.

During the Crusade the Custodions acted as emmisarys of the Emperor, there isn't any fluff peices of them acting as bodyguards for others.

As of now however the Custodes remain in the Imperial Palace and only venture out into Terra to bring people to the Emperor.

The Custodes protect the Imperial Palace (the size of a continent) in general in addition to the Emperor. That includes everyone in it.

Not entirely correct. During the Crusades, etc. After the Heresy the Custodes retreated to the Inner Sanctum only to focus on protecting the Emperor. In any case, even during the Crusade era, the Custodes protected the Emperor and the Emperor alone. The protection of the 'Palace' was defense in depth, and though one would presume that the protection of the Palace meant an extension of that protection to those inside, this is not the case. If killing everyone else in the Palace meant protecting the Emperor then it would be a justifiable and valid response to the situation. The IMPERIAL FISTS are the ones who protect the Imperial Palace (or did anyway, not sure if they maintain that duty after the Heresy).
