Dreadnought leg armour looks pretty good as a shoulderpad. I don't think I have any pics right now but I've seen it and it looks pretty good if done right.

I honestly don't know. You'd probably need to either slice something up or use a blob of greenstuff. I'll see if I can find a tutorial for you.

There are a few third party vendors out there who make the "pre-heresy" type shoulderpads. Shopping around should result in a number of styles to chose from depending on the exact look you're going for.
