Is the radius of effect of a Blood Chalice or a Teleport Homer 6" from the model with that wargear? Or is it to the unit that has the model that has that wargear?

i.e. If Brother Corbulo is standing in the back, aside from the unit which he is with (M), does X get it?

From the model with that wargear. It is not something granted to the unit, but granted to the model carrying the wargear.

THe exception is page 66, as when embarked you would measure to the hull which is alsocongruous with the unit.

I believe you need to be within 6" of the model carrying the blood chalice/ teleport homer to gain the effects as they do not give permission to measure from any model in the squad to check range.

6" from the chalice model, or his transport if he is mounted up.

It is from the model, Other wise you could just have a long line and pass the bonus onto you entire army.
