If I have a librarian that has been pinned while in a crater would he get his 3+ cover save against a mindstrike missile?

No as you do not use cover saves, you use Invulnerable saves for Perils.

The have a normal strength value, right? Just have a side effect of causing Perils?
I could be wrong - away from my books. If all they do is cause Perils, then you don't get a cover save.

Against the normal missile wounds, you can take a cover save (though your armor save is better, so you must take that). Against the perils, you only get your invulnerable save of 5+ which you must re-roll if successful

the potential to, yes.
One from perils, one if the S4 missile wounds you. You take the best save available in each case, which cannot be cover for Perils because, as Perils specifies, you dont get cover saves gainst perils of the warp
