Impact miniatures have a single hockey player model at the moment, i remember reading over on TMP that the owner was planning on releasing a full range of modern hockey players sometime in 2012.

The PJ's not the greatest, but it looks like a decent enough sculpt. It might be a little too futuristic looking if you're trying to do modern players though.

JT Scott had a winter-sports/Christmas themed CSM army many years ago. Not sure where he got the hocky sticks, but he had two five man squads of CSMs converted to hocky players; with blades under their feet, and using the berserker legs and assault marine legs to get the kicking/moving poses.
