Alright well the point if this thread is to ask that, if the blade of Nurglitch is used in combat by a character, normal models would be killed if they suffer a wound negating the swords affects(as most only have one wound) . But in a combet with units with models with more than one wound how do you differentiate the model with -1 to their toughness

You dont - when you wound units of multiple wound models the wounds are floating unti there is enough to kill a RnF model. So attack a champion, and you reduce the toughness, but not otherwise
