Lychguard must *all* have the same equipment in the same unit. MSS causes a unit to *hit* with D3 attacks, so you cant miss, but armor saves are allowed

I had no idea about this! He has always run them with mixed Weapons. Also I probably wasn't specific enough, but I rolled for 2 attacks and neither got a 4+ to wound. So yea, they hit but they didn't do any damage. Could have been more clear though I suppose.

Edit: Also, my Banshees would have gotten no Armour saves as it says that you take the attribute of the CCW of that unit, since it was a Banshee, it was a power sword. Otherwise spot on.

Your 10 man crusader squad should not have assaulted the loota squad in turn 2. Units may only assault the turn they disembark if the vehicle has an assault ramp or is open topped.

This was a 5th Edition game, not 6th. In 5th you could do this as long as the Transport had not moved before they got out. You are right about it in 6th edition.
