Sheets are pretty common from actual hobby shops (not game stores). Most will carry one brand or another in the 4 primary flavors (clear Inkjet, clear laser, white Inkjet, white laser). Hobby Lobby also carries the Testor's brand sheets. The above link also provides a few online choices.
Beyond the specific choices for the paper to match your printer - you will need to have a bit of understanding for the design. Most people end up not being entirely satisfied with the results because they use a raster design program (Photoshop for example) at low resolutions and as a result the printing isn't crisp enough.
I do custom decal sheets for all of my stuff - miniatures and models, and when I put them together in Photoshop...I have the resolution cranked up to 1200 dpi. Most the time though, I will actually make the design in a vector based program (Inkscape is pretty good and free). This allows you to scale them as needed for things like shoulder markings and vehicles.
After all that - you will need to print it of course. Double check your printer to make sure it is up to par (have it run its cleaning cycle...you don't want to print a sheet at maximum resolution only to have a streak down the center due to a dust bunny). When you put the sealer on (I use regular rattle can clear coat) lots of thin coats. If it pools on the paper, it will cause the ink to run unless you are using a laser printer.
Make sure you have a good sealing coat on the sheet. If not, it will turn into a technicolor mess when you try to do the water transfer (again - not a huge issue with laser printers).
The other option is to use a printing service. There are several who do custom decal sheets. I know Ginfritter's used to do them, though that was before they merged with Armorcast...so I am not sure if they still do.
