If you were going to recommend a book to introduce someone to the world of 40k what would it be. I've only read the Horus heresy stuff and don't think they're a good intro as they expect some knowledge already. What do you think?

Not sure about the HH as its not "present day"
I would say to get the theme behind 40K you can't go wrong with Helsreach:
http://www.blacklibrary.com/all-products/Helsreach.html
and it also introduces the reader to pretty much all the elements of the Imperium:
The Astartes, Sisters, Mechanicus, Guard, civilians, Priesthood and the relationships betwen them. Orks
and its a damn good read...............

I AM A MARINE PLAYER
"Unimaginably ancient xenos artefact somewhere on the planet, hive fleet poised above our heads, hidden 'stealer broods making an early start....and now a bloody Chaos cult crawling out of the woodwork just in case we were bored. Welcome to my world, Ciaphas."
Inquisitor Amberley Vail, Ordo Xenos
"I will admit that some Primachs like Russ or Horus could have a chance against an unarmed 12 year old novice but, a full Battle Sister??!! One to one? In close combat? Perhaps three Primarchs fighting together... but just one Primarch?" da001

The Gaunt's Ghosts series by Dan Abnett, and Daemon World by Ben Counter.

I should have left him there. He had served his purpose. He owed me nothing - yet he gave himself to me willingly. Why? I know not. He is nothing more than a pathetic human. An inferior race. A mon-keigh. But still I broke off my wings so that I might carry him easier. I took him from that place, into the snowstorm where our tracks will not be found. He is heavy. And he is dying. And he is slowing me down. But I will save him. Why? I know not. He is still warm. I can feel his blood ebbing across me. For every beat of his heart, another, slight spill of heat. The heat blows away on the winter wind. His blood is still warm. But fading. And I have spilled scarlet myself. The snow laps greedily at our footsteps and our lifeblood, covering them without a trace as we fade away.
'She sat on the corner, gulping the soup down, uncaring of the heat of it. They had grown more watery as of late she noted, but she wasn't about to beggar food from the Imperials or the "Bearers of the Word." Tau, despite their faults at least didn't have a kill policy for her race.' |

Little known fact: Bobakos is a clone of my DNA, but only 1/8th my size. I concur with his assessment.
However: If you are trying to get someone into the fluff with their first book, my recommendation has to be Let the Galaxy Burn. Not the Horus Heresy novel, but the short story compilation. It's a massive tome of 40k short stories, edited by Marc Gascoigne and Christian Dunn. Since it covers a broad range of authors, armies, and themes, it is the perfect way to start reading about 40k. It allows you to pick the authors or genres you want to read more about without investing all that money into a full length novel only to find out you hate it.
Much like any kind of cuisine, a sampler platter is probably one of the best ways to familiarize someone with new stuff.

"Cadian Blood" is the book which I've always been able to get someone introduced to 40k with.
It's a fast read, it's not too heavy on concepts--and for a "Zombie Apocalypse" novel it's pretty in-depth.
