I'm pretty sure the answer is no, but I would like to clarify either way.

Can the SM Librarian take 1 power from the codex and 1 from the new disciplines in the rulebook?

FAQ states you generate 2 new powers from the 4 disciplines in any combination instead, so I'm assuming it's either all or nothing.

If he exchanges his powers, he must exchange all of them.

Then, you can mix between the rulebook disciplines.

Of course, he can still take smite and one from the rulebook, since smite is a primaris power.

Just to clear that up, he can always have smite if you want. Choose smite and another power from the C:SM powers for your list but switch out for random powers during set up.
Since smite is a primaris power you can trade your rolled power in for that from that table.
