can every model armed with krak grenades use them in the assault phase?

Yup. With 1A each.

Krak grenades can only be used against vehicles and monstrous creatures in close combat; it's one attack per model in the unit that has it as a part of their war gear at the S, AP listed in the BRB.

(Ex, 10 Grey Hunters are assaulted by a carnifex. They pass their counter-attack check, but instead choose to use their krak grenades. They proceed to roll only 10 attacks at their WS and the S of the 'nades vs the 30 attacks at their WS and base S.)

Also, one dude in the unit during the shooting phase may throw a krak grenade.

He can throw it when overwatching the carnifex too.
