How do people do it? I know you are generally at a disadvantage for this, but is it plausible? I am thinking of doing a grenadier one with 2 12 team platoons, 1 normal, as well as heavy mortars, antitank guns, antiaircraft, and infantry recon. Basically I am thinking of doing the classic counterattack strategy, with a little more offensive punch. I have two concerns with this list though.

What if my opponent just sits back with his artillery and bombards me, not commiting any killable resources to me. I can't go up the board or I will be easily killed by all of his guns. My heavy mortars would be easily destroyed if my antiaircraft guns don't take out his AOP, and how am I going to deal with all the spotters US have?

Medium tank lists- If I move out of my trenches, they would be on me like a pack of dogs immediately after I left the proctective aura of my AT guns. Because of their large numbers, my relatively large number of hand held AT might not be enough without AT support

Recon- I really hate recon- always lifting my GTG by forcing me to shoot at them or loose my GTG. They also could harass my infantry with impunity. Maybe add a light armor platoon to shoot and then get the heck out of dodge?

Large blobs of infantry unwilling to attack me- my platoons rely on the machineguns to tear apart assaulting infantry with the help of my nests. My machineguns would be insufficient of the enemy is well dug in and numerous.

Back in the English morass

Fortified companies need a lot of artillery; they can counterbattery fire to subdue enemy artillery, knock out or push back enemy recon and they are a big threat to enemy infantry who will be forced to do something.

Medium and light armour lists are always difficult to attack with infantry companies, there isn't really much that you can do to counter them other than taking armour of your own.

The prefect example of someone missing the point.
Do not underestimate the Squats. They survived for millenia cut off from the Imperium and assailed on all sides. Their determination and resilience is an example to us all.

Its just a shame that they couldn't fight off Andy Chambers.

Tried a fortified FJ Cassino list once.

Best I can offer is this.

1. Have a definative goal. Just getting out of the trenches to take the fight to them is never a good idea. The scenaios typically will be set up so they have to come to you. Use that.

2. If you get out, smoke becomes your best, and sometimes only, friend. Two light mortar sections, or mortars and nebs are ususally best rather than going to heavy mortars.

3. If the mission/opponent looks like you will need to get out to win, placement of your emplaced elements (pillboxes, HMG nests) becomes key. I found a couple games I was pushing my AT guns up a bit further than comfortable knowing I would probably have to use them to cover a dash out of safety.

But really...smoke. As much as you can comfortably bring without compromising the rest of your list. That, and if possible, vets over trained.
