Checking out the new Sisters of Battle (excuse me, Adepta Sororitas) codex, I came across the Ecclesiarchy Relics.
It's clear you can only have 1 per model (unlike the Space Marine relics) per army.

Checking the page where all of the other relic points cost are located, the Sacred Banner seems to be missing both an entry and points cost.
The only other entry I see for the Sacred Banner in the PDF is under the Sororitas Command Squad options.

Does this mean the only model/unit that can take the Sacred Banner is the Sororitas Command Squad despite showing up in the Eccesiarchy Relic list (which would allow other characters to pick it up)?

Marines also have a "relic" banner that isn't listed in the wargear lists where others relics have their point costs.
The only way to take it is on command squads or honour guards. I suspect it works the same for the sisters

Correct, only Command Squads can take it. The Reliquary Ministorum is just rules information, the presence of the Banner there has no bearing on who or what can take it . Who can take what items is defined by the Wargear section and each unit's Options section.

This may have already been covered, but what about the +1 Attack? Does that work in the shooting phase and in the assault phase, or does it only give an extra attack in CC?

It modifies the Attack characteristic,so melee only. If it modified shots it woukd say so like the DA Relic Banner does.
