It is not set in stone.
There are arguments for both opinions (same with grav-gun)
But if i remember correctly, the latest version of the official online builder of the SM codex allows for 5x grav-guns and apothecary upgrade in a command squad.
Perhaps one who has the ipad version can confirm this?

I thought the latest version did the opposite. If you had a 5 man command squad, and you gave all 5 men 2 grav-guns each then converted one man to an apothecary it would reduce the number of grave guns in the unit down to 8 and not let you move them back up to 10.

I have been pondering this myself....and while I'm still on the fence, at this time I will take the Ork FAQ as a precedent for allowing the apoc to have any vet upgrades carry over during his upgrade to apothecary on the basis that all the upgrade does is add a nethicarn to his wargear and change his name.
the ork entry I refer to is:
Q. If a Boyz mob exchange sluggas and choppas with shootas, can a
Nob take a power klaw or a big choppa? (p100)
A. You may upgrade the Nob to have a big choppa or power
klaw before you choose to upgrade the mob to have shootas, in
which case the Nob is not affected by the mob’s weapon swap
(as he no longer has a choppa to swap), does not receive a
shoota and keeps his slugga and power klaw/big choppa
instead.

How about checking the appropriate question in the Ork FAQ which does not allow an Ork Painboy to take 'eavy armour.

It's also worth noting that the Red Scorpions chapter tactic that turns tactical sergeants into apothecaries makes specific mention that they can still take sergeant wargear.

my reference is more to do with the difference between upgrades.
there are no rules for the timing of upgrades and or equipment and how they interact with each other when bought as part of a package.
for example:
two veterans, one buys a melta gun and then buys the apothecary upgrade, the other just buys the apothecary upgrade. does the first one loose his meltagun despite the upgrade not replacing any weapons? true, but what is the wording of that particular upgrade from Nob to Painboy? is it that he replaces, upgrades or does he generate extra equipment to become a painboy?
in the command squad a veteran is only given a piece of wargear and a name change, he isn't replaced, looses no other wargear and started out the same as his buddies

The painboy question is the appropriate precedent. Its more than just a timing issue. The question you're trying to use fails to take into account that the model changes its name and profile when upgraded. When upgraded, a model may only legally have the options its new name and profile permit regardless of what options you gave it before. An Ork Nob upgraded to a Painboy can't have options that are only available for Nobs and a Space Marine Veteran upgraded to an Apothecary can't have options that are only available to Veterans.
