It's not on CMoN, and I can't find it here on dakkadakka either, but wow I saw this through google it is an exact match of the picture of Azrael in the codex, some really amazing work in my opinion, and I'd like to see more of this artist but this is the only one they have listed.

No idea, but if you scroll down on the page on the left side there is a comments section and it appears he/she is answering questions on it.

The name of the artist on the profile is Kyriakos Simos which is Greek. If you need to find and contact him maybe I can look him up in our wargaming community .
