There is something odd in that picture. I don´t think it is the Emperor.
I mean, he looks... alive. Perhaps it is an astropath or something, aping the Emperor to bolster his capacity, I don´t know. May be it is fan art?
Compare with this:

He is a rotten carcass, or at least in a really, really, really bad shape.

As always, I assume we see the setting through legends and tales.
So these are artistic concepts based on half-remembered stories about The Time of Ending, drawn at some point during, say, year 50k  . No wonder they look odd. Of course, maybe it is the hand of another person.
