Ok, a character gets two failures, an advantage and a triumph (would have been 3 failures but for the triumph). What happens? My group is really stumped.

This happened twice in a row with a grenade throw, we had the grenade roll downhill and catch the closer guy in the blast, but we weren't really sure...help?

A Triumph counts as a success so it would just be one failure with advantage. Overall it was a failure so it probably wouldn't damage the targets, but it may have made them lose cover or coherency a bit due to the advantage. Since it was a triumph in combat the player could have spent it to gain different bonuses either to himself or allies (page 206 of the rulebook).
