A friend of mine was playing an Assassin and through a series of buffs and debuffs had a to hit of +35. He rolled around talking about how he was about to unleash the pain ect ect, and then proceeded to roll a 1.

Amidst the mists and coldest frosts he thrusts his fists against the posts and still insists he sees the ghosts.

More an epic fail for the creature itself. We were fighting Yeenoghu and his minions and because of his high magic restistance I keep summoning creatures in. A ghoul I summon in proceeds to get three ciritical hits and kills him off. One of the great princes of hell banished by a mere ghoul, not something we expected.

Paladin in plate mail takes a critical spear hit to the chest (12 points of damage) and dies in a random ambush before getting to the dungeon

In the same adventure the Fighter runs into pit trap charging towards kobolds. Then climbs out of pit, on the kobold side, only to turn and run, falling back into the pit, when kobold reinforcements arrive.

Later still in the same adventure the Thief sneaks ahead and locks himself in the final room with the Kbold King ans hi Heram. The thief swiftly defeats the more powerful Kobold King, only to die being beaten to death by the 1/4 hit die Kobold Heram.

Even worse, thief and bard outisde of the room listen to the battle through the door, and decide the party should staty outside untill the racket dies down, not knowing who was on the opposite side of the door.

Sword of Sharpness, we houseruled that on a botch roll (1) you would cut yourself. The party found a Sword of Sharpness unguarded in the middle of a dungeon and left it behind, along with 2 of their hands and a foot.

At GenCon there was a guy playing a Gnome Paladin in plate (+8 to AC) armor. In 4th Gnomes move 5 squares base but armor has a -1 penalty, so he only moved 4. He literally could not keep up with the battle. Even a Dwarf moves a minimum of 5. To top it off, Gnomes have a race specific feat to wear Chain (+6 AC) and Scale (+7) without movement penalty and gives them a +1 to armor against large creatures. He also had a weird build to. Went WIS/CHA but took abilities that used the STR as a modifier.
