Loss: It was in a battle of my SOB against Necrons. It was near the end and we were tied for points. My sister squad was exchanging fire with a Necron warrior squad and I needed to kill them all because my opponent had great luck with all his resurrection shenanigans and if I didn't wipe that squad out, it would just be back up to strength and my sisters couldn't win in a battle of attrition. So, all 15 of them fired and I got freaking almost all ones! Crap! (I should figure out the probability of that.) Now I'm going to lose that squad of Sisters.
Win: I was about to let out a stream of curses, but then I remembered that I had my act of faith and rerolled those misses. All hits! I wiped the squad out, leaving his Necron lord exposed. St. Celestine then swooped in and cut him to pieces.

A long time ago, late 2nd ed or early 3rd ed, my dad had a Vindicator that got tracked. He open fired on empty ground next to a eldar unit, can't remember what it was. Nothing in the rules said you could not shoot open ground so he did, rolled the scatter die and it landed straight in the middle of the squad. Killed most and the rest ran come his turn.
Then about 4th ed, my brother charged a FW Baneblade my buddy just bought, back then, and killed it with a single chain fist. Catastrophic damage and blew it up. Then my buddy unloaded in rage into that squad with about 20 lascannons. Needless to say the termies all died, but took a Baneblade with them.
The small fun I had was also in 4th ed. We were playing a small game, like maybe 750 points. I had a bike marine army and was getting chewed up by my enemies landspeeder. I charged it with my chaplain on a bike and melta bombed it killing it. Then the rest of my bikes, not many by this time, just rolled through and ate up the rest of his army.
On a none 40k moment, I lost a game of fantasy in the first turn when my friend routed my whole army off the table when he shot one of my Clan rat blocks with an Organ gun. Quickest game I ever had. Took longer to put the rats on the table then it took to lose.

me and my friend where just starting our 40k he was using the assault on black reach box set as space marines. is was space wolves it was 500 points. my list was one 9 man squad of grey hunters with a plasma gun banner power fist and plasma pistol. a rune priest was coming with them they were in a drop pod. my other squad was a 6 man grey hunters with banner and plasma gun. is was objectives he deployed first his entire army against my 6 man squad. My drop pod did not come in tell the last turn  my 6 man squad was whipped out against his hole army. by the time my pod came in it was to late.

Win: Dumping a 5 man DWT squad with a Chappy from a LRC into a 5 man crisis suit unit, completely destroying them. Next turn everything else in the 1500 point Tau list shot at them, killing NONE. They proceeded to demolish the Tau gun line.
Fail: Ezekiel (with prescience) charging a 1 wound Zoan and getting killed, awarding my opponent Slay The Warlord, and beating me by 1 victory point.

On Nimbosa, cramming as many guardsmen into troop carriers as possible.

Having my Imperial Guard commander with powerfist survive 3 rounds of assault with a terminator armored chaos sorcerer and kill him. It was amazing.
My biggest loss would be my all plasma cannon devastator squad overheat and kill themselves in one turn of shooting.

Both of mine come from 5th edition.
Win: Lone Khorne Berzerker Champ charges five SW Terminators. Opponent wiffs all of his non-PF attacks. My Champ lands 5 hits, wounds on all of them. Five failed invulns and he is left standing. Consolidates, survives incoming fire from long fangs + RP thanks to cover, charges the Fangs and wipes them thanks to No Retreat. Captures their backfield objective securing me a win.
Loss: Running a full GK Terminator army at 2k points versus BA. Dice decide to lay down some hate. I fail every single armor save the first two turns. After 15+ failed saves i basically have a LRC and a Librarian left. I concede and give away my dice dice to my brother. He proceeds to use them to shoot with his ten man sniper scouts in the game he just started. Rolls 9 hits and 8 rends on a Carnifex, thanks me for the hot dice, and goes on to table the nids.

4 Lootas Overwatching a Tzeentch DP with Wings, Power Armour, the whole shebang. Was down to 3 wounds from the same guys gunning at him earlier. He is yet to do anything except fly straight towards them.
4 hits, 4 wounds, 2 2's and a single 1 which was re-rolled into another 2. Suffice to say my opponent was not pleased. Not pleased at all.

Ork v ork battle tonight.
Only thing I have left is a off mek on his own, down to one wound, and facing a ton of firepower, plus an unhurt burnabomma.
His lootas down the bomma, and it crashes onto the mek, killing him... classic, awesome and orky...

My biggest surprise win was in 2nd Edition with my chaos vs a nids army. He has a ton of little guys, a carnifax(which is not synapse, not sure if it is now) and a Hive Tyrant in the middle. I have my line set up across my deployment zone so he cant hit everything at once. I have a vet squad with an autocannon guy with a clean LOS to the Hive Tyrant, fires a shot, the first shot of the game. I think both hit and wound. Fails his armor save, so I roll the number of wounds it causes.(2nd you rolled to hit and wound, then they do the armor save, then you roll for how many wounds it causes).Does just enough wounds to kill it. Entire army is now out of synapse and randomly wanders off, player concedes.

If you dont short hand your list, Im not reading it.
Example: Assault Intercessors- x5 -Thunder hammer and plasma pistol on sgt.
or Assault Terminators 3xTH/SS, 2xLCs
For the love of God, GW, get rid of reroll mechanics. ALL OF THEM! |

Biggest Loss: I return to the game after 14 years, first battle with my wolves I take turn one, movement phase, Njal the almighty son of Fenris caller of Storms begins to gather his sorcerous energies to cast the warth of the tempest, psychic check double  I swear to Morkai that this won't happen again I end my movement phase and Njal begins again to chant in a low voice to put forward the Jaws of the Wolf World, psychic check, double  BANG!!!! Warlord death in turn one without the enemy needing to drop a bullet on him, I almost ragequitted the battle and the game...
Biggest Win: A lone Grey Hunter survivor obliterated 2 full 20 adepta sororitas Squads, he's been transferred to the Wolf Guard and his Saga's just begun.

My mega OP veteran sergeant (kitted out with a power fist and storm bolter) and his squad were assaulted by a daemon prince, and the sergeant was challenged, i honorably acceoted as i usually do with him, and he proceeded to take off all four wounds and kill him in 2 turns of fighting, then went on to assault a 20 man cultist squad with only 1 other veteran (with power sword and storm shield) and killed 13 before they were killed... epic win!

Won a game where my friend failed a 3 inch charge 3 times. Also used a buffmander single riptide combo to blow up 3 necron vehicles with a single shot before it was cool. then proceeded to lose almost my entire army, then won because my kroot where painted the same color as a hill that was in the middle of the table that had 1 objective. won by like 2 points.

Fail: I gave my opponent First Blood and Slay the Warlord on MY first turn, and I was going first.
I was running Space Wolves using a Rune Priest as my warlord, and I kicked off my very first turn by casting some spells. I cast Prescience, roll the dice...double 6s. Well, that sucks, but it happens. I cast Perfect Timing, roll the dice...double 1s. Aside from the pre-game rolls, I literally rolled 4 dice and gave my opponent two free points because of it.
Win: An Ork army wiped out a line of Imperial Guard tanks by assaulting them with a lone Big Mek...which had been catapulted across the table on the first turn with a Shokk Attack Gun.
Not mine, but it was one I saw a while ago. The game was Guard vs. Orks, and the Guard were running a line of tanks. The Ork player's HQ was a Big Mek with a Shokk Attack Gun and a Power Klaw (he ran it that way because he was sick of it constantly getting assaulted). He rolls first turn, and fires the SAG at one of the Guard player's tanks, rolls double 5s for his roll, and immediately assaults the tank. It then spent the entire rest of the game just plowing through the Guard player's line of tanks with his Klaw. No matter what the Guard player threw at it, it just couldn't die - it either passed its cover saves, or it was in such a position that most of the Guard army was out of line of sight, or the attacks against it simply missed/scattered/failed to wound.
