I am working on a Green Tide list with the added restriction of only the formation and a single FOC type detachment are allowed. What I am thus struggling with is which HQ choices to include for which buffs, and then whether to include a 3rd HQ via the ork horde detachment instead of the regular CAD.

I see the following special rules as things you want the detachment to have:

Fearless (from either Mad Dok or Big Bosspole)
FnP (from either a painboy or Mad Dok)
+1 WS (from the Lukky Stikk)
Infiltration (from Finkin' Cap shenanigans)
Psychic Defense (from just throwing a Weirdboy in the unit)

At present I have the Big Bosspole on the Green Tide's Warboss. There is no other relic from the Ghazghkul supplement that interests me anyways. That said, what else would you include? And would you use only 2 HQ choices sticking to a CAD or would you use 3 HQ choices via the OHD?

I actually played this formation for the first time yesterday againist chaos. I used mad Dok and a warboss with lucky stick. If it wasn't for FNP and fearless that squad would've died by the 3rd turn. It lasted right till the end of turn 6. I never got to test the lucky stick as my opponent used a phyicic power that made every model in the unit attack him, But that WS 5 would of helped alot. I would suggest as many power klaws as you can as well incase someine throws a walker in that your boys cant kill.

Depends on the point limit. I'm thinking of a wierdboy just to be able to counter the annoying telepathy madness and have at least some chance of denying invisibility on something like spawns who can otherwise tarpit your 100 guyz all day long.
