So, how exactly does one do this?

The thing about 40k is that no one person can grasp the fullness of it.

How do you mean? You mean slicing off the pistol grip so you can mount it on top of a closed hand? Just kind of slice the grip off. It's thin, and not too much trouble. I try to leave the trigger guard on, if possible.

Stage one: Pick up knife, preferably hold the non-sharp end. Stage 2: Apply knife to pistol. Choose carefully which part to remove, for example, be careful as the cylindrical section is the barrel, not the grip! Stage three: separate the pistol and grip, again being careful to remember which is which, and dispose of the unwanted section. Stage 4: carefully place knife on table, not into unwanted locations such as your own face or an electricity socket.

Really, grab the knife, cut carefully and then your done. I'd consider pinning it to the hand, as otherwise it'll have a tendency to snap, which is annoying.

I usually use a flush cutter

but nowadays its already mounted to the hands so is less of an issue.

The GW plastic snips work good for me, I cut off a large chunk of the handle then file down and glue onto the hand.

I've always had more trouble with swapping a bolt pistol (already attached to arm) and another weapon without destroying the arm or pistol.

I cut away the gun with flush cutters, leaving 0.5-1mm still attached, before carefully shaving off what's left and cleaning up with a sanding sponge.
