Exactly as the title suggests. For example: Warp spiders have the Deathly Web Shrine, Shade Gate Shrine, and Slicing Skein shrine (just naming the ones in the codex pictures). Which Craftworld(s) is each one on? Is this even covered in the fluff?

All of them.
Some craftworlds have more or less devotees of a given shrine(such as ulthwe having more reapers, or Iyandan having very few of any).

Shrine of Midnight Lightning: Shining Spear shrine
Shrine of the Dark Moon Waning, Cold Death, Ending Veil: Dark Reaper shrine
*CONTINUITY ERROR*: Ending Veil is described as the shrine of 2 of Korlandril's friends, yet later it is said it is the Shrine of the Enduring Veil.
Shrine of the Star of Justice, Light that Burns: Dire Avenger Shrine
Shrine of the Rage of Khaine: Fire Dragon Shrine
Shrine of the Maidens of Fate: Howling Banshee Shrine
Shrine of Hidden Death: Another Scorpion Shrine
Shrine of the Fall of Dark Rain: Scorpion shrine but more aggressive than the Deadly Shadow which is more stealth.
Shrine of the Deadly Shadow: Scorpion shrine which encompasses an area of jungle/swamp and has a plant covered ziggurat.
From WD 323 in 2006:
Shrine of the Falling Moon (Dire Avenger shrine)
"Named for the cursed moon of Eldanesh, the Dire Avengers of Alaitoc are veterans of a protracted war against the Imperium"
This shrine is again shown in the latest Eldar Codex.
From Apocalypse War Zone Valedor:
The Shrine of the Patient Blade, famous upon Iyanden and Biel-Tan alike for their excellent tracking skills...

The above suggests there is a shrine of the Patient Blade on both Iyanden and Biel-Tan.
From Apocalypse War Zone Valedor:
The Ascendant Echo shrine (Howling Banshees) took to anointing themselves with alien ichor in the early stages of the DUriel Campaign, daubing the rune of the Howling Banshee on their bone-coloured armour in the hope of attracting the favour of Khaine.
Aluinnthralann, Exarch of the Strangling Veil shrine, came to prominence during the battle for Iyanden, where he used his warp jump generator to leap vertically into the path of a Harpy. Locking his powerblades into the beast''s carapace, he reduced the creature''s head to a fine red mist before teleporting away once more.

I think there are many shrines for each / most Aspects on each Craftworld, with Iyanden having fewer generally and also Ulthwe, as they focus more on psychic and Guardians. Thing with Ulthwe is that there surely can't be THAT many psychers as there would be Aspects so I'd put them between Iyanden and the rest of the Craftworlds in number of shrines.
Wonder what state Altansar is in...

I believe certain craftworlds lack certain shrines, for instance Shadow Specters are a fairly rare on even most major Craftworlds.
I think the idea behind the naming of shrines is how that specific shrine operates. So, when craftworlds have the same shrine that share the same name cross craftworlds? And perhaps they just took the same name due to the fact that they follow similar skills and teachings?
Painting should never be a restriction! For my Craftworld fluff I named a Firedragon shrine "Shrine of the Spears of Vaul" and painted them a dark blue with a red helmet to kinda hone back on their origins as firedragons.
Sky is the limit!!

It's likely that Craftworlds may have multiple shrines of the same aspect. Incubi, albeit Dark Eldar, also have shrines. Automatically Appended Next Post: Which of course, can be located in Commorragh, or in the web way. I wouldn't be surprised if they had any in the open galaxy as well. As I gathered from the novel "Path of the Incubus"

Let me put on my robes and vase helmet... *cough* Each Aspect upon a craftworld keeps at least one shrine in which to practice the mastery of their Warrior Path. (Codex: Eldar. "The Path of the Warrior". 6th Edition. P10. print) Some Aspects, such as the Slicing Orbs of Zandros, are unique to a specific craftworld. Others are common to most, with the most famous and well-established being the Dire Avengers, the Howling Banshees, the Striking Scorpions, the Fire Dragons, the Swooping Hawks, and the Dark Reapers. (Codex: Eldar. "The Path of the Warrior". 6th Edition. P11. print) The Forgeworld Shadow Spectres Aspect seems to reaffirm that there had been a few lost Warrior Aspects, which means the players would have been able to make their own. But GW never did; although it's probably very hard to balance and they'd get pissed off due to copyright infringement. The Dark Eldars Inccubus do maintain Shrines dedicated to Khaine, but they are not aspect warriors since, well, they don't follow an Eldar Path, they are just... axe crazy... Turns out you wanted to know which of the named shrines are on which Craftworld... well the pictures are just there to give the players an excuse to paint their aspect warriors differently from the Craftworld army they have. Call them whichever, a craftworld is huge, maybe an Eldar Carpentre changed to the Aspect of the Striking Scorpions just because there's one such shrine situated close to where he lives.

The Dark Eldar certainly do follow a path to become who they are, they have a twisted version of the same practices.
It's unknown if a Klaivex is the same as an Exarch in becoming multiple personalities.
Although they don't have the same system as Eldar, its affectively the same idea.

@NauticaKendall Nah -- the Eldar Path is actually a system developed to damper the Eldar's hyper sensitive minds and easily distorted desires by restricting their pursuits to one goal at a time, and through the path a Craftworld Eldar will gradually learn to give up all the excessive pursuits that brought about the birth of Slaanesh. The idea behind an Eldar path is fundamentally different from the almost Khornate bloodlust (as is, ironically, befitting Khaine) that is the worship of the Incubus. To ensure temptations is put behind them, the philosophy often called Ai'elethra, or the Path, governs every aspect of craftworld life, enabling the Eldar to harness their emotional and intellectual intensity safely, without jeopardising themselves or those around them. (Codex: Eldar. "The Eldar Path". 6th Edition. P10. print)
The Incubus are twisted mirror images of the aspect warriors, but they are by no means operating under the same idea. They literally just want to be the perfect killing machine, and will do everything they can, and kill everything they have to in order to to achieve it.
