In the entirely hypothetical situation in which every Avatar of Khaine that is or ever was were summoned simultaneously in the same place, would their power still be separate because each avatar is a separate entity? Or would the warp god be reformed?

You'd need a special ritual to reform Khaine, and even then he'd immediately revert to the warp and get ganked by Chaos all over again.

They are avatars, they contain tiny fragments of khaine and his martial rage but no, they would not reform into a god. Theres not an inkling of fluff to that.

Khaine could be restored. For an example look at Qah, the Old One God of the Hrud. Like Khaine, Slaanesh shattered him and broke him. Each of his shards formed into an Umbra, which congregate around the center of late stage Hrud civilizations where the join together to make more powerful Umbra. Similarly, something is directing the Hrud to attract these Umbra, so we can only assume even shattered gods can have some level of interaction with their followers.

However, what you should realize was that Khaine was not a nice god. He held no love for the Eldar by the end of the War in the Heavens, having already attempted to slay them all to spite Isha. If Khaine was to be reformed, it would not be the best interest of the Craftworld Eldar, however i can perfectly imagine that the Incubi sects would be up for trying to reform him.
