When one buys the combi-weapon upgrade (i.e., combi-flamer, combi-melta, combi-plasma) for a CSM rhino from the Chaos Vehicle Equipment list, does it replace the existing combi-bolter or is it in addition to the combi-bolter?

Take doesn't mean replace, so it is in addition to standard equipment.

CSM is a 6th edition codex. With 6th edition they brought back the armory concept in the form of specialized lists that multiple units could reference instead of having a single line item for each option. In this case, as I am not to familiar with CSM, you will need to look in one or two places to get your answer. If the option to buy the combi weapon is via a single line option then if it replaces a weapon it will tell you in that line. If the option is part of a table like the vehicle equipment list the need to replace a weapon will be mentioned just above the list of prices.
