You should have established this before you start your game. I've seen some players allow it if it's at least an inch and some allow it if it's just a centimeter. All cover should be agreed on before the game (like trees being soft or dense).
B) do I have cover from enemies on the ground only?
You draw imaginary lines (or get a laser pointer) between the two bases. If at least one can connect the two, there is line of sight. If at least one crosses terrain that you've set as cover and the target is close enough to it, that cover goes into effect.
C) is the wall ignored because it's within 1" of the edge?
Are you referring to a vantage point here? That's another thing that has to be agreed on at the start of the game.
