The rules for Magma cutters state the following:

If a Maulerfiend hits with at least one Attack in the Fight sub-phase, it may make an additional Attack with one of its magma cutters against one of the models it hit.
If it hit with all of its Attacks, it instead makes two additional Attacks with its magma cutters against one of the models it hit. These attacks hit automatically and are resolved at Initiative 1.

If a maulerfiend hits with all of its attacks, does it:
A) make one attack with each of its magma cutters?
B) make 2 attacks with each of its magma cutters (so 4 in total)?

I'm leaning towards B because if it was A, I think the wording would have stated that it made 1 attack with each of its magma cutters (similar to the first part of the rule).

As far as I'm concerned, it's A. You get one bonus per cutter.
if you hit with one, you get to make 1 additional with 1 of the cutters. So if you hit all, you get 1 additional with 2 cutters, for 2 total.

Actually, I'm pretty sure it's A now aswell. It says "it" (meaning the maulerfiend) makes 2 additional attacks, using the magma cutters as its weapon.

Also 4 attacks would be a bit too good for it's points cost I think.

btw do you also get a free melta attack on even a 1 wound model if you hit it but fail to kill it?

You just need to hit to generate potential wounds from magamcutters.
So even if you only hit once, you will roll to wound twice. Once for the normal hit and once for the magma cutter.

Of course the model might not be around anymore for the magamcutter to do anything as it hits at initiative 1.

You'd think so, but think about it, a weaponskill 3 model hitting 3 (4 on the charge) attacks? It's not THAT likely and it could only target models that already took a hit and survived. I don't know a whole lot of models that would survive 4 Str10 AP2 hits unless it's something that has both a 3++ or better and eternal warrior (or T6+).

Or land raiders.
