Is it possible to run a Heavy Weapon AND a Shield on the same model? e.g Plasma Cannon (+15) and Storm Shield (+10) for +25 Points?

If the model has the options to do so, yes. I don't know of any units or models with this combination of options, though.

Dark Angels Company Veterans, or have I misread the codex and only one model per squad (not all) may have a storm shield? If the case is that only one per squad can take it could it be the heavy weapon model?

To quote the codex:

Any model may take one of the
following:
- Combat shield…5 pts/model
- Storm shield…10 pts/model

then yeah you would be allowed to, as it doesn't switch out a weapon, it is just an add on

If Company Veterans also have access to the heavy weapons list then absolutely, yes. They don't have to swap anything for the Storm Shield so it's taken in addition to whatever other options they've selected.
