My best challenge: Weirdboy VS Warboss with Power Klaw
We were playing Orks VS Orks. A challenge could have been issued by the Weirdboy, but he didn't want to. So the Warboss issued a challenge. Weirdboy missed all 3 force weapon attacks, and Warboss got him with 4 power klaw wounds.

My worst challenge: Ork Warboss with power klaw VS Tyranid Prime with a pair of boneswords
Warboss issued a challenge against Prime with a pair of boneswords and rending claws. Warboss missed all power klaw attacks, and suffered instant death when the Prime rolled 2 .

What about you?

Best Challenge: Bloodthirster challenging and murdering everyone, including a Necron Overlord.

Worst Challenge: Terminator Chaplain with Relic against a kitted out TWC Chapter Master. Those guys are way too strong with up to 5 Relics on one guy.

Best Challenge: UM Tactical Squad Sergeant against a Tyranid Prime. Prime misses with all of his attacks, then my Sergeant swings back with his power fist, hits both times, wounds twice and instant deaths the Prime. Pimp Slapped that boy!

Worst Challenge: UM Chapter Master (kitted out) vs. Dante. Dante swung first, hit with every attack, and wounded with every one. I failed 4 Storm Shield saves (in a row!) and died.

My best? There are two of them:
1, me vs my SM playing opponent. He charges a unit of 10 Guardsmen with his Vanguard, all kitted out with Relic weapons and his Sarge issues mine with a challenge. I accept and..... the puny human proceeds to blow the Vanguard Veteran Sergeants head clean off. The look on my opponents face was worth the rest of the section getting massacred.
2, My certain SM playing opponent has brought a new homebrewed character whom would turn out to be of near (if not at) primarch power levels. Said character spends most of the game running around, wiping out two Chimeras and a veteran section before hitting my Command Section. He issues his challenge and I accept. He grins a gak eating grin and proceeds too..... Whiff every attack bar one. Against all the odds my faceless vanilla bod of a commander (whom is on one wound) passes his 5+ invulnerable save and proceeds to butcher this god level character. Needless to say this was epic enough but then said Company Commander proceeded too (on his own) destroy a Vindicator, survive a round of shooting and overwatch from a Sternguard squad and then butcher said Sternguard too.
The butthurt was with my friend that night.

Free from GW's tyranny and the hobby is looking better for it

Best and worst are both Inquisitor-related.

Best: kitted out SM Captain challenges my force-sword wielding inquisitor. He has something unwieldy, but is not too worried due to having artificer armor. A single Force wound gets through and the 170ish point captain is ID'd by a 75 point Inquisitor

Worst: my Inquisitor challenges an Eldar Autarch, who whiffs all but one attack, causing a single wound. He strikes back and fails to wound, then gets swept when he fails his leadership test.

Best: Back before our new codex, my Warboss and his Nobz charged into the disembarked Kor'sarro Khan and his retinue after their Land Raider was wrecked by a double 6 SAG shot. Kor'sarro narrowly avoided killing me outright since he rolled 2 6's to wound against my Warboss but I managed to roll two 6's in return for my invuln! My Warboss then promptly proceeded to snip his head off with his PK, with Kor'sarro having failed to pass any of his invuln. saves against my attacks back.

Worst: It's either Angron murdering my 2 Warbosses and Ghazghkull in a simultaneously challenge between the 3 (Thanks to his Lord of the Red Sands rule) or my Weirdboy getting stomped to death by a

I think the issue is that the Prime itself is actually T5, its what puts him above the rest of the Tyranid Warriors. So that shouldn't have insta-gibbed him.

Worst is drazar he always fluffs either attacks or wounds and has faied to kill anything at all.

Best is nightbringer (newest version) pimp slappin a smash fether chapter master in one round. Of course he was then shot to death by bolters but oh well.

Probably my favorite was in 6th ed, where berzerkers with chainaxes charged a small group of grots. Runtherd challenged, because why not.

Berzerker champ has like 5 attacks, goes first, hitting and wounding on 3s. No saves, needs one wound to kill. Hits with only two and fails to wound.

Runtherd rolls double 5s to hit and to wound (which he needed to wound), and champion fails his armor saves (maybe both) and goes down.

Grots, naturally, get completely wiped out, but wounds didn't carry over to the runtherd (6th). He still lost combat by a bunch, but rolled snake eyes for leadership.

Was rescued next turn by meganobz and, I believe, survived the battle. He was pretty heroic.

Best: Tau Crisis commander with fnp, shield generator and fusion blades in combat with Skull Taker and Kharne. Slaps Kharne with the fusion blades, then the next turn accepting Skulltskers challenge and boxing him out after the blade break.

Best Challenge: Sammael vs Forgefather Vulkan He'stan. Vulcan charges I was able to ship off a wound with over watch from Sammael. Long story short we both kill each other in combat.

Worst: Ravenwing Sargent vs Chaos Chosen. Missed all of my rolls just was a bad night.

It wasn't really a challenge, but it was glorious anyways! An Imperial Knight with 4 hps assaulted my chapter master Smashfether, he failed to wound my chapter master and proceeded to lose all 5 hull points through glances and one pen.

A Protoss colony world

Best: I was running a 6th ed. Dark Angels list with Azrael as my warlord. My opponent was running Necrons (this was like three months after they got their strong codex), but not a Decurion. Azrael drop podded in near the opponent's Overlord and Lychguard group. Overlord and Lychguard charge, Overlord challenges, Azrael accepts. Azrael chops down the Overlord in one round of combat, although he didn't live to brag about it for very long (died to some pissed-off Lychguard).

Worst: My Dark Angels Assault Veteran Sergeant with a Power Axe got boned hard by a Khorne Daemonkin Juggerlord. Initiative 1 is a bitch!

Best: Daemon prince charges my group of firewarriors, has 1 wound left after overwatch. Whiffs all his attacks against the Shas'ui... who proceeds to bonk him to death with his one attack.

Worst: Vulkan and a drop pod full of sternguard pod in, roast some guardians. I decide to assault them with my Wraithlord, to at least kill a bunch of Sternguards. "I challenge your Wraithlord!" Oh, right... Wraithlords have the Character rule, so they can get locked in challenges. Vulkan spends the next two turns making all his invulnerable saves, while the sternguards slowly pick away at the WL with crack grenades.
