On the RT book cover, what is the meaning of the shoulder icon on the marine using the ork head as a grenade? It is also on the 25th anni. crimson fist figure(from RT).

Do you mean the eagle with the (human) skull? I think it's just some early artwork of an Imperial Aquilla. Could be wrong, though!

Under the couch

The winged skull is known as the Imperialis. It was originally adopted by the Loyalist Legions during the Heresy, but later became an honour badge denoting righteous victory.

This may be a retcon, but the Imperialis symbol has recently been shown off as being a symbol during the Heresy-era awarded to units who have suffered a great amount of casualties, yet keep up the good fight for the Emperor and Mankind.

I'm not sure if that actually is an Imperialis, subtle difference I know but every mention I've seen of the Imperialis has been a skull with wings, rather than an actual bird with a human skull.

In that particular placement however it was the RT rank emblem for an Imperial Commander, which is an Adeptus Terra rank which in 1st edition was held by Imperial Planetary Governors and Space Marine Chapter Masters.
