Well Krieg is dangerous for just being so monumentally toxic, but throw on your Death Korps standard issue re-breather and chemically treated trench coat and you're set for most surface excursions.
Catachan on the other hand, no equipment can protect you. It's not like you can just take a precaution and be safe because everything is trying to kill you. Quite actively.
So I'd say Catachan.

Except for those carved Catachan abdominal muscles!
But yeah, if wearing true armor is so irrelevant that troops canonically don't even bother with it, I'd think that says a lot about the nature of their environment. Furthermore, the whole "every known species of flora and fauna are predatory, poisonous, or both" thing does it for me.

Im going to second Catachan, its only export is beefy insane soldiers because it can only sustain a population with outside imperial help, and that population barely survives long enough to become those beefy insane soldiers. Its an entire economy built around dying. It dosent get more grimdark than that! Plus I remember something about having to use flamers to clear the jungle around their village every few days otherwise the jungle will reclaim the town. I'd also give Fenris a close second in this category simply because they are doing all their survival with iron age equipment as opposed to getting supplies from the Imperium, even if their planet is *slightly* less likely to murder you. Edit: I almost forgot Baal! On the moons there they live in a mutant filled toxic Mad Max world that every generation they gather up hundreds of children and make them do battle to go away with sky gods.

Fenris. Planet can freeze you to death, all fauna is dangerous, and the main society on the planet are (duh) Space Wolves, who seem pretty brutal from their fluff. (I have not read too much fluff on them though)

I think Catachan makes a strong case, with Fenris (where not just the animals but the entire environment, including its volatile tectonic plates, are more than willing to murder you for existing) fairly close behind.
The planet Murder from the Horus Heresy Series deserves a call-out (When Space Marines are getting diced up like IG, you know its a death world of the highest caliber).
