Adrian Smith. Think it first appeared in White Dwarf in the mid-late 90s. That's certainly where I saw it first.

You're misremembering. That particular piece didn't appear until the early 2000's in the Art of Warhammer 40,000 book and was given away as a poster in White Dwarf (same issue as the Old World map IIRC) later on.
