So has anyone else done this? (Stupid question as I've read loads of people say they have/will) as I have no idea how to do this well. I have three spare MkIII marine legs and bodies that I plan on converting to apothecaries with power swords for my Death Guard. Would the 40k apothecary pieces work?

I'm in the same boat but without the 40k pieces.

The 40k narthecium pieces would absolutely work, the only real difference between early and late mk arms is the forearm section which is completely hidden, so just stick one on, do the rest of the model in 30k parts, give it an appropriate paint scheme and Bob's your uncle!

I've got my own conversion handy if you want a pic, but it's pretty straightforward.

Pics would definitely help. I've just gotta find some apothecary parts now!

Apothecary and a master of signals:

I carefully cut the vials off the belt, and transferred them to the Mk. IV torso. The rest is just 40k era command squad bits and BaC Mk. IV.

The MoS has BFG antennas and a targeting arm from an old CML terminator.

Still awaiting paint.
