Had a brief google with no luck - I don't think it's a standard GW one (but may be wrong on that), so my bet would be Forgeworld or a custom made one.

It seems to have a lot of similarities with Captain Titus' chainsword from the Space Marine video game.

Yea that's what I was thinking. Looked on FW and couldn't find it. I'm betting the guy that made this took a regular chainsword and converted it to a relic chainsword using greenstuff.

I believe its the one from the sternguard kit.
