So in 8th, where a codex entry gives you the option to "replace x with y" is it free? Do you pay the difference? Do you pay the whole cost all over again? Specifically I'm wondering if I have to pay 12 points to replace a 14 point Force Staff that's included with a Librarian with a Force sword.

I'm pretty sure you just buy the final wargear that ends up on the model, you don't have to pay for weapons you swapped out.

The points values pages tell you whether a model's points per model includes wargear or not.

The Librarian's point value does not include wargear, so you would choose your wargear and add that value to the Librarian's.

You pay for the final wargear, I think that's pretty clear from how it's set up and how the prices turn out.
e.g. a Predator costs 101 base, 49 for a Predator Autocannon which is it's stock weapon. You can exchange it for a Twin Lascannon which costs 50, so basically the same.

Predator autocannon: 48'' Heavy 2D3 S7 AP-1 3 Damage
Twin Lascannon: 48'' Heavy 2 S9 AP-3 D6 Damage
2 Lascannon Sponsons: 2x 48'' Heavy 1 S9 AP-3 D6 Damage

Twin weapons are literally just 2 regular weapons glued together and technically inferior (can only target one thing), but GW prices them the same. It would be crazy for the Twin Lascannon to effectively cost 99 points when the sponsons are 50 in total.

Side question to this. For Dark Angels, Belial costs 150. This says including war gear. If I upgrade him to TH/SS, does he stay at 150 or does he pay the extra 40 (190 total)?

He stays at 150 because his points are "including wargear".
