So on a Carnifex you can replace each pair of Scything Talons with "Two Deathspitters" (From Wargear section). Do you pay for each Deathspitter, or is the points cost listed in the back of the book for each pair of deathspitters. Basically Do you pay 2x the cost or 4x the cost that is listed if you take all weapons as Deathspitters?

You would pay 4x the cost listed in the back.
You're buying two guns per slot, and the cost listed in the back is for a single weapon.

Yeah it seems odd at first glance, but it does give some flexibility.

It's not a "Twin Deathspitter" it's 2x Deathspitters (obvs with Slimer Maggots).

"Twin" weapons often come at slightly reduced cost in other Indexes as you have to fire both barrels at one target.

The DakkaFex with four guns doesn't have any "Twin" limitations, so could target four separate units if it wanted.

I don't know, how can we be 100% sure of the intent? I mean, the rules say you pay for 4 and get 4, but how can we be certain they didn't mean to get two twin ones?

Everything that has a 'twin' weapon has a distinct profile for it. A separate statline that has double the shots.
For instance a heavy bolter with 3 shots, and a twin heavy bolter with 6 shots.

That's not the case here. There's only one profile for the gun, and an entry in the list of upgrades that says you get two of them.
