So the Ironstrider Ballistarii has a BS of 3+, it can take a twin Cognis autocannon which is Heavy 4.

Cognis allows me to advance and fire but I must subtract 2 from hit rolls.

Since the weapon is Heavy I would have to subtract again so I'm hitting on 6s right?

Correct. The unique rule's penalty is cumulative to any other penalties, such as the one for moving and firing heavy weapons. Keep in mind that any other -1 to hit modifier is going to leave you unable to hit.
