I picked up the codex and noticed there is a Primaris Apothecary but no regular one. I take it I can’t field a regular one?

That's correct, given I believe the Apothecary was not an option for Deathwatch in Index: Imperium I either.

It's kinda bizarre, but you don't actually lose much only having the Primaris one, seeing that the original wasn't customizable. You'd only lose out on transports and that's it.

No Model, No Rules.
It wasn't even an option in the Index, so I can only assume it was never an option.

Yeh I wanted to put a Captain, a Veteran Squad, and an apothecary in a Blackstar and use it as a major hitter but....

Yeah I find it confusing that you can't have an apothecary or techmarine for DW when you could literally have the model just by slapping a deathwatch shoulder on one. That's literally what they do with the Primaris version and it totally works.
