For the Volcano Lance which has 3D3 damage. WHich of these is it?

or D3+2 , D3+2 D3+2 so (d3+2)*3 rolls.

Is it adding 2 to each die roll or the total at the end.

It doesn't say "Add 2 to the damage roll of each die," it says "increase the damage characteristic by 2."

If you add 6, you're not doing that.

It says increase the damage by 2, not 2 per die. You'd do 3D3+2.

I think is 3D3+2 since it is a single attack

It's not 3(D6+2) it's (3D6)+2, as per the rules for modifying characteristics.
